           infection control and quality preventative dental care. We believe the best
           dentistry is the least amount of treatment necessary to achieve optimum oral

           We understand the needs of our patients and take great pride in our work. Feel

       APPOINTMENT TIMES

           We respect your time and make every effort to remain on schedule and ask that
           you extend the same courtesy to us. If you are unable to keep an appointment,
           please notify us at least 48 hour prior to your appointment so that this time may
           be given to another patient. There may be a cancellation fee if this policy is
           violated.

        REGULAR VISITS

           At Centrum Dental Centre we believe in the benefits of regular preventative
           care, and, therefore we encourage all of our patients to return for periodic
           cleaning and exam visits. The interval between these visits varies from person
           to person and depends on a number of factors including level of gum and bone
           disease and general health. For your convenience we can pre-book your next
           recall and then follow up to confirm the appointment we have scheduled for
           you.

        FEES

           At Centrum Dental Centre we follow the Ontario Dental Association’s
           Suggested Fee Guide for General Practitioners.

        INSURANCE AND PAYMENTS

           At the end of your appointment, we will present a statement of the services we
           have provided and the amount you owe for these services. Payment is due
           when services are rendered.

           Payment methods available are cash, debit, VISA and MasterCard. On your
           behalf we will electronically submit a dental claim form to your insurance
           company. If your insurance provider does not accept electronic claims, we will
           assist you in filling out the appropriate paper claim form.

           We will only recommend treatments that are necessary for you continued
          dental care. Please be aware that dental insurance coverage helps you to cover
          your dental needs. It does not dictate your dental needs. Only your dentist can
           diagnose dental problems and only he or she can recommend care based upon
           those needs.

           Dental insurance has not kept up with modern progressive dentistry in terms of
           costs or services offered. As a result, your insurance provider may not always
           cover necessary treatments that are recommended by your dentist.
